Dermal Fillers

There are so many options for facial fillers these days and we are bombarded by these options all over the media. You see their ads on the television, hear about them on the radio, and read about them in your favorite magazine.  So, what’s the real story?  Basically, the goal is the same – to smooth away unsightly facial grooves and wrinkles – and just the actual products vary slightly.

What is the difference between Botox and facial fillers?

Here is the rundown of the dermal fillers Dr. Spiegel offers at his Boston plastic surgery center:

Restylane® and Perlane®

RestylaneThese injectable fillers are both a form of hyaluronic acid, which is found naturally in the body. The product is placed in the dermis to gently fill in lines and grooves on the face.  Restylane® and Perlane® are perfect for the lips and nasolabial folds but can also be used in the marionette lines, cheeks, and many other areas. Restylane® typically lasts 4-8 months and Perlane® results last longer—anywhere between 8-12 months (without a touch-up treatment).  Many patients elect to have a touch-up treatment about 6-8 months following their initial treatment and see their full correction for over a year!  Perlane® is made from a denser formula that can be injected deeper into the face and takes longer for your body to break down giving you a longer lasting, natural-looking result.   The formula consists of tiny particles suspended in a gel – think tiny “jello-y” particles in a smooth gel that is ideal for grooves in the face.  Both Perlane® and Restylane® have been approved by the FDA for facial wrinkles and contain lidocaine for a more comfortable treatment.   Some swelling is expected following the treatment and typically lasts for 12-48 hours. Some patients experience bruising following the treatment but this is minor and resolves itself over a few days.  Makeup can be worn to disguise any minor bruising.

Juvéderm®

JuvedermThis product is from Allergan Inc., the makers of Botox and is also made of hyaluronic acid.  Juvederm® is most commonly used for the lips, nasolabial folds, and marionette lines. Some people say that the product is slightly smoother than some of the other fillers on the market – like a smooth gel – and prefer it for use in the lips.  There are two versions of the Juvéderm®, Ultra and Ultra Plus.  Patients find that Juvéderm® Ultra typically lasts 6-8 months while Juvéderm® Ultra Plus can last more than 12 months (most results vary between 8-12 months). Juvéderm® has been approved by the FDA for facial wrinkles and comes in a formula with lidocaine!   Similar to Restylane® or Perlane®, swelling and/or bruising are the most common side effects usually lasting for 12-48 hours following the treatment.

Radiesse®

Radiesse Wrinkle TreatmentDr. Spiegel was one of the first physicians in Boston Massachusetts to offer this material for the treatment of facial folds and wrinkles. Radiance is a form of calcium hydroxylapatite – a natural mineral in an aqueous gel.  The product helps your body create its own natural collagen!  This FA-approved material can be injected through a fine needle and provides a soft, natural improvement that can last 1-3 years! It’s perfect to give your cheeks a boost and to fill in the nasolabial folds. Radiesse® also comes in a number of different sizes offering patients more options with the amount of “fill” they require.

Is an injectable material called Poly-L-Lactic acid that is especially effective in treating areas of fat loss with gradual treatments.
